With the bases loaded and two outs in the top of the ninth inning in a tie game against the Baltimore Orioles, the Philadelphia Phillies completely fell apart at the seams.

Engineering the absolute worst mistake of the season so far, a routine infield fly turned into two runs for the Orioles as nobody had a clue what to do. Check out this ghastly faux pas, as three different players refused to claim this pop fly.
